Distance to Care: A Resort Metric Rarely Priced
Distance to care is the time between an acute event at a property and the handover to a facility that can treat it. Appraisals price the beach and the airport, not this. The desk read official and primary documents for Nungwi, Gili Air, Uluwatu and Lake Como, graded what it found, and says where it found nothing.
IllustrationKey takeaways
- - Distance to care is the time from an acute event to handover at a facility that can treat it, and standard appraisals do not price it (Victaura Research definition).
- - Nungwi lies about 56 km north of Zanzibar Town, a distance proxy and not a transfer time (directional, encyclopedia-reported, Wikipedia).
- - The Mnazi Mmoja Referral Hospital was reported 29.8% complete, with completion targeted for November 2027 (directional, press-reported, The Daily News).
- - Tumbatu Island had 18,366 residents at the 2022 census, and its planned health centre is to take more than 30 inpatients and have an operating theatre (directional, census figure press-reported by The Daily News, 29 December 2025).
- - Foreign visitors to Zanzibar must buy inbound insurance from the Zanzibar Insurance Corporation for stays up to 92 days since 1 October 2024 (directional, regulatory notice, Tanzanian embassy site).
- - At least 1,291 tourists had left the Gili islands on six ships by 7 August 2018, and Mataram's hospital reported shortages (directional, press-reported, Jakarta Post and Hurriyet Daily News).
- - Menaggio recorded more than 10,000 emergency visits in 2018, and its emergency room status was contested in June 2024 and January 2025 (directional, press-reported, Quotidiano Sanità and Il Giorno).
- - The desk found no official transfer time for any of the four places, and the Uluwatu and Gili Air cells are not graded (Victaura Research, 27 October 2026).

What is distance to care, and why does Nungwi make it visible?
Distance to care is the elapsed time between an acute event at a property and the handover to a facility able to treat it, and Nungwi shows why the kilometres understate it. Nungwi lies about 56 km north of Zanzibar Town (directional, encyclopedia-reported). That is a distance to a town, not stated as a road distance, and not a transfer time to a hospital. No ministry document the desk could open gives the distance to Mnazi Mmoja Hospital itself, so the 56 km is a proxy and is labelled as one.
The metric has four parts, and each fails in its own way. Recognition is the time before anyone decides the event is serious. Stabilisation is what the nearest clinic or the property's own staff can do. Transport is the vehicle, the road, the boat or the aircraft. Reception is whether the destination can take the case at that hour. A resort can look close on a map and still lose time in three of the four.
Valuation rarely carries the metric, because nothing in a standard appraisal asks for it. Appraisals price location by distance to the beach, the airport and the town. They do not price the hour a guest might need at three in the morning. The omission is not malice. It is a gap in the template. For an allocator underwriting a hotel room or a villa, the gap is a risk that sits outside every line of the model until the day it is used.
This article measures places, not projects. It reads official and primary documents for four locations, Nungwi, Gili Air, Uluwatu and Lake Como, and says where no such document was found. The result is uneven. Two places have usable press reports, and Nungwi also has an embassy notice. Two have a general government advisory and little else, plus, for Gili Air, one historical event. The unevenness is itself a finding, because the principal meets the same fog when asking the question on the ground.
Which hospital serves Nungwi, and what is changing in Zanzibar?
Zanzibar's main referral hospital, Mnazi Mmoja, is being rebuilt, and the rebuild is not finished. The Citizen reported on 4 September 2025 that Zanzibar's Second Vice President had announced on 2 September 2025 that the existing buildings were in the final stage of demolition, to make way for a modern referral hospital. Mnazi Mmoja is one of two primary referral facilities in the islands, with Abdallah Mzee Hospital in Pemba (directional, press-reported).
Construction progress is the number that matters to a principal. The Daily News reported, in a piece that appeared in late October 2026, that Zanzibar's Minister for Health targets completion of the Mnazi Mmoja Referral Hospital by November 2027, and that the project manager put construction at 29.8% complete (directional, press-reported). The report names the Saudi Fund, the Kuwait Fund and BADEA among the financiers.
What the rebuild does not say is how an acute case reaches it from the north. Neither article mentions ambulances, emergency department capacity or the north of Unguja. The government's stated aim is to treat locally the patients previously sent to Muhimbili National Hospital on the mainland or to India. That tells an allocator where severe cases have gone until now. It is a statement about the referral ceiling, not about the first hour.
A second tier is planned around the referral hospital. On 31 December 2025 allAfrica reported plans for three referral hospitals with staff housing, the new Mnazi Mmoja, a cancer hospital at Binguni and a teaching hospital at Binguni. The Citizen reported a plan to build ten district hospitals, and regional hospitals under construction in all five regions. Plans are not capacity. The base case should assume today's network until the buildings open and are staffed.
Tumbatu, a foundation stone and an admission
The most specific evidence of local capacity on Unguja's north coast concerns a facility that does not yet exist. The Daily News reported on 29 December 2025 that Zanzibar's Second Vice President laid the foundation stone of a Tumbatu Health Centre. Tumbatu Island, in North Unguja Region, had 18,366 residents at the 2022 census (directional, census figure as press-reported by the Daily News).
The planned services are the useful part. The report lists outpatient services, maternal and child clinics, emergency and accident care, diagnostics, laboratory services and surgery with an operating theatre, with a capacity of more than 30 inpatients and 24-hour operation. A foundation stone is a promise. A principal should record it as one, and ask for the commissioning date from the Zanzibar Ministry of Health rather than from a press release.
The same report is also an admission. It says residents of Tumbatu face risky sea crossings to reach medical services. If a resident population of 18,366 lacks a nearby facility with surgical capacity, a visitor on the same coast cannot be assumed to have one. This is not a criticism of anyone. It is a calibration of what the word near means in the north of Unguja.
For Nungwi, the working assumption is a clinic tier nearby and a hospital tier in Zanzibar Town. The desk could not open a ministry list stating which facility serves Nungwi, and says so. The principal should ask for the name, opening hours and surgical capability of the nearest facility in writing, and should not rely on a map pin.
A map pin shows a building. It does not show an operating theatre, a night shift or an ambulance with fuel.

Zanzibar's compulsory visitor insurance is a floor, not a plan
Since 1 October 2024 foreign visitors to Zanzibar have been required to buy inbound travel insurance from the Zanzibar Insurance Corporation, for stays of up to 92 days. The rule is stated in a notice published by Tanzanian embassies (directional, regulatory notice read through the embassy site and not the official gazette). Visas News reported on 1 October 2026 standard rates of US$44 for adults and US$22 for children aged 3 to 17 (directional, press-reported). The desk did not open the policy wording.
A compulsory policy sets a floor, and the wording decides what it floors. Whether it pays for air evacuation, for a transfer to the mainland, or only for local treatment is a question for the policy document, and the desk asserts no limit. The Tanzania health page of the UK Foreign, Commonwealth & Development Office (FCDO) says medical facilities are limited, especially outside Dar es Salaam, and advises checking that insurance covers local treatment or unexpected medical evacuation.
For owners and renters the question splits in two. A guest arriving from abroad carries the compulsory cover plus whatever policy they bought. A resident owner or a long-stay tenant may sit outside the visitor scheme, which the notice addresses to foreign visitors, and needs private cover that names evacuation. The operator should be able to say which case applies to which guest.
The emergency number is a small fact that is rarely tested. The FCDO page for Tanzania gives 112 and asks callers to request an ambulance. Whether a call from Nungwi reaches a dispatcher with an ambulance in range is something a web page cannot show. The principal can test it by asking the operator who answers, in what language, and with which vehicle.
Gili Air, where the sea is the road
Gili Air has no road to a hospital, and the desk found no official document that states its transfer time. Searches for the Gili Indah health centre, the regency health office and any sea-ambulance arrangement returned nothing usable. The figure is therefore not graded, and the article offers none. A transfer passes by boat to Lombok and then by road to a hospital, and each leg depends on sea state and on a boat being available.
The only documented stress test is the earthquake of August 2018. The Jakarta Post reported on 7 August 2018 that at least 1,291 tourists were evacuated from Gili Trawangan, Gili Air and Gili Meno, using six ships, five of them private and one from the state operator PT Pelni (directional, press-reported). Hurriyet Daily News reported on 6 August 2018 that a hotel receptionist on Gili Trawangan said there were not enough boats.
The hospital end of the chain was saturated as well. The same Hurriyet report quotes the Mataram general hospital's spokesman saying the hospital needed paramedics and medication, and says hundreds of injured were treated outside damaged hospitals. A disaster is not the base case, and the desk does not present it as one. It shows that a boat dependency and a regional hospital cluster can be strained at the same moment.
The FCDO's Indonesia page supplies the general frame. It says the standard of local medical care can be poor, that some tests cannot be performed reliably, that treatment for serious illness may not be available in remote areas, and that medical evacuation can cost tens of thousands of pounds. It does not mention Lombok or the Gilis. The principal should read it as the floor of an assumption, not as a finding about Gili Air.
Uluwatu and the nearest equipped hospital
The desk could not retrieve an official register entry that fixes the nearest equipped hospital to Uluwatu, so no distance is stated. Searches for Ministry of Health and BPS-Statistics Indonesia Bali Province (BPS Bali) hospital material returned nothing usable in this pass, and the commercial listings that surfaced are not primary documents. The Uluwatu cells in the table are therefore not graded.
The principal can generate the missing figure by test, and the test is cheap. Name three hospitals with an emergency department that could receive a serious case. Confirm each by phone, and in writing, for hours, imaging and paediatric cover. Then drive each route from the property at the hour a night call would be made, and record the time. The result is a measured figure owned by the principal, which is better than any broker's description of near.
Class matters more than proximity. The nearest facility may be a clinic. The nearest facility able to operate may be farther away. A listing that says a hospital is nearby has not said which of the two it means. The question for any seller or operator is the capability of the facility, its hours, and which guests the operator has transferred there before.
Traffic and tide are variables, not constants. Road time on a coastal peninsula changes with the hour and the season, and the desk has no measured series for it. The metric should be stated as a range with the hour attached. A single figure with no hour is a brochure number.

Is the emergency room on Lake Como staying open?
The last report found says yes: on 15 January 2025 Il Giorno reported that the Region had ruled out closing the Menaggio emergency room, after a June 2024 report that it would close. The Erba-Renaldi hospital at Menaggio is the lakefront facility. On 11 January 2019 Quotidiano Sanità reported the director general of ASST Lariana, the health body that took over the hospital, saying it would remain an acute hospital with a 24-hour emergency room, after more than 10,000 emergency visits in 2018 (directional, press-reported).
The status then moved twice in seven months. On 22 June 2024 Il Giorno reported the regional welfare councillor saying that Menaggio would lose its emergency room and become a hospital for chronic patients, and noted about 14,000 signatures collected against the change. On 15 January 2025 Il Giorno reported that the Region had ruled out dismantling the emergency room, again on the councillor's word (directional, press-reported).
As of 15 January 2025, per Il Giorno, the emergency room was not to be closed, and the desk found no later primary notice. A principal should treat that date as the last verified point and confirm the current position with ASST Lariana before relying on it. The pattern is the lesson. A service that is promised in 2019 and questioned in 2024 cannot be priced as permanent.
The wider system includes a 118 control centre and a helicopter base at Villa Guardia. Quotidiano Sanità describes a base for the province of Como that houses the 118 control centre, the helicopter emergency service, the continuity-of-care doctors and five ambulances, replacing the earlier base at Erba (the article's own timestamp reads 2011, so the details may have changed). FCDO's Italy page gives 118 for an ambulance. The lake road and the ferry timetable are not documented here.
Older guests and small children, where distance to care bites
The guests most exposed to distance to care are the ones a resort most wants to attract. Older guests carry chronic conditions that turn a minor event into an emergency, and families with small children need paediatric capacity that a general clinic may lack. A property that cannot name its nearest paediatric-capable facility has not answered a question these guests ask.
Medication continuity is the quiet risk. A guest who loses a prescription on an island may not find the same product within reach, and the operator is often the only local contact. The desk has no data on pharmacy coverage in any of the four places and states none. The principal should ask what the operator does in that case, and who pays.
The operator's emergency protocol is part of the asset. It names who decides to move a guest, who calls, which vehicle or boat is used, and who meets the guest at the destination. It should be written, tested and shown. The same discipline applies in choosing a property manager, where the first question is rarely about marketing.
Rental demand has a reputational tail. One badly handled transfer can generate a review that outweighs a season of good ones. This is directional reasoning, not a measured effect, and the desk offers no figure for it. It is cited because the downside is asymmetric, and the allocator should read an operator's protocol as a hedge against that tail.
A resort that cannot say how long it takes to reach an operating theatre has told the principal what it has not measured.

Insurance and evacuation, what a principal should ask before buying
Insurance is the instrument that converts distance into money, so its wording is part of the underwriting. The FCDO's Indonesia page says medical evacuation can cost tens of thousands of pounds. Its Italy page says a Global Health Insurance Card or a European Health Insurance Card is not an alternative to travel insurance, and lists medical repatriation to the UK among the costs it does not cover. These are advisories, not quotes.
The first question is whether the policy covers evacuation or only treatment. Evacuation means moving the patient to a better facility. Repatriation means moving the patient home. One does not imply the other. The wording, the sub-limits and the named assistance company decide it, and the desk has not read the policies of any insurer for these places.
The second question is who authorises the move and how fast. Some policies require the insurer's assistance company to approve a transfer before it happens. On an island at night, that approval step is a time cost. The operator should know the call procedure and the policy's approval rules for the guests it hosts, including renters who arrive with their own cover.
The third question is the owner's own position. An owner who spends part of the year at the property needs cover that is not tied to a short visit. Property insurance and health insurance are separate products, and neither answers for the other. A principal who holds the asset through a company should also ask who bears a guest-injury claim, a legal question for counsel, not for this desk.
How should distance to care enter an underwriting model?
It should enter as a scored checklist with a date, before it enters as a number. The desk has too few official transfer times to price the metric, and a made-up figure would be worse than none. A five-line score is enough to start. Name the nearest clinic, the nearest hospital with an emergency department, the transport mode, the measured daytime and night-time transfer, and the evacuation cover. Date each line.
The cost sits in operating lines, not in the purchase price. A written emergency protocol, first-aid training for staff, a standby vehicle or boat, and higher insurance premiums all show up in the operating budget. The principal can ask the operator for these lines. If they are absent, the metric is unfunded, and the yield on paper leaves out a cost nobody has booked.
Remoteness is also the thing that is sold. The same distance that lengthens the transfer protects the setting, which is the logic in environmental constraints as an asset in resort property. The two are linked, not opposed. Scarcity of access is part of the scarcity of supply, and the allocator who wants one inherits the other.
Planned hospitals should enter as scenarios, not as the base case. A hospital that is 29.8% built or a health centre with a foundation stone changes the picture only when it opens, is staffed and is reachable. A sensible model keeps the current network as the base and lists the planned facilities as upside with their reported dates. That keeps the model honest if the dates move.
Honestly disclosed, what this article could not establish
The article found no official transfer time for any of the four places. The brief asked for hospitals with emergency departments and transfer times from official sources. For Nungwi the desk has a hospital and a distance proxy. For Gili Air and Uluwatu it has no register entry and no official time, and the table says so. For Lake Como it has press reports, not a health authority register.
Several sources are old or undated. The Menaggio statement is from January 2019, the Villa Guardia base article carries a 2011 timestamp, and the FCDO pages show no date. The desk read them on 27 October 2026 and cannot say when they were last updated. Each should be reopened before use, as the methodology on this site requires for any dated fact.
Some inputs are press-reported only. The 29.8% progress figure, the Tumbatu details, the 2018 evacuation numbers and the Menaggio visit count come from news reports of statements, not from the underlying documents. The Zanzibar insurance rule was read from an embassy notice, not the gazette, and the price came from the press. The grades above reflect that.
The article contains no valuation effect. It does not say how much value a shorter or longer distance to care adds or removes. Nobody can say from the evidence collected, and the desk will not invent a discount. The claim is narrower. The metric is real, it is rarely priced, and the principal can measure it before signing.
The desk has a conflict of interest in these markets, set out in the disclosure below and on the methodology page. Readers should weigh the commentary accordingly, and should check each cited document themselves.
What the allocator takes from this
The allocator should ask for distance to care as a measured, dated line in every island or lakeside underwriting. It costs little to produce. It needs a named facility, a tested route, a written protocol and an insurance wording. It does not need a forecast. Where an operator cannot supply it, the absence is information.
The four places differ in what can be said, and the difference is the lesson. Nungwi has a referral hospital under reconstruction and a distance proxy. Gili Air depends on boats, and the only documented test was a disaster. Uluwatu has no retrievable register entry in this pass. Lake Como has a network, a helicopter base and a contested emergency room. None of the four can be priced from a map.
Renters and guests deserve the same clarity as owners. A guest with a child or an elderly parent is entitled to know how the property handles an emergency before booking. An operator who publishes the protocol makes the guest's decision easier.
Skin in the game disclosure. Victaura, through its parent Greystone B.V. The Insurance Layer: When Climate Risk Gets Priced
Insurance is the fastest mechanism through which climate risk reaches a price. The property market reprices a coastal asset over a decade; the insurance market reprices it every twelve months. Major carriers have stopped writing new homeowner business in exposed US states, parametric products fill part of the gap at the frontier, and a transaction discount on sea-level-rise-exposed homes has been measured in US data. For island and coastal luxury real estate, insurability has become an underwriting variable of the same rank as title.

Environmental Constraints: The Scarcity Premium
Building moratoria, height caps and protected-area statutes are usually read as headwinds. The record across several resort jurisdictions points the other way: a binding, tested limit on new supply becomes a floor under the value of everything already built. The constraint does not eliminate risk. It relocates it from the market to the statute book, and a statute book that has survived a court challenge or a referendum holds longer than most underwriting models assume.

Island Utilities: Water, Power, Waste Costs
On a mainland site, water and power are a monthly bill. On a small island, they are a piece of process plant with a capital cost, a design life and a failure mode. This piece prices what that plant actually costs to build and run on Zanzibar's coast, and discloses where the data runs thin.
